Welcome to the BlackPlanet Site for Catawba Care Coalition.

Catawba Care Coalition (CCC) is a 501(c)(3) not-for-profit organization located in Rock Hill, South Carolina. CCC was formed in 1994 and was originally housed at the Rock Hill Health Department and operated under the United Way. In 2000, CCC expanded to include more case managers and an on-site medical clinic. To facilitate this change, the agency incorporated, received status as a 501(c)(3) non-profit organization, and moved to its current Rock Hill facility. Since 2000, CCC has continued to expand and merge with other agencies to alleviate duplication of services and maximize limited resources. This growth include a 2001 merger with the York County AIDS Task Force and a 2003 merger with the Catawba AIDS Prevention Network for consolidated prevention, outreach and education services.

Our mission is to promote optimal health for York, Chester and Lancaster County residents by providing medical care and support services for people living with HIV/AIDS and by educating the community to prevent the spread of HIV. We serve clients in one convenient location with: Medical Care at The Christopher Clinic Case Management and Support Services Prevention Services

Adults who are HIV+ and living in York, Lancaster or Chester counties can receive medical care at The Christopher Clinic. Our clinic staff includes:

• Medical Office Assistant
• Registered Nurse
• Nurse Practitioner
• Infectious Disease Physician
• Medical Case Manager
• Mental Health Counselor

Our prevention staff works to reduce the spread of HIV by providing education to the community and conducting HIV testing and counseling. Other services offered through the Prevention Department include:

• Help with disclosure
• Safer sex strategies
• Safer injection strategies
• General HIV Info STD/Pregnancy
• Prevention Group Education Programs
• Community Outreach
• Free Condoms

Our case managers help clients with many needs. To receive services,clients must complete an evaluation with the Intake Case Manager. Our support services include:
